How to Enroll in the Foreign Service Benefit Plan

Employees - There are a few ways to accomplish enrollment in the Foreign Service Benefit Plan (FSBP) depending on your circumstances.

  • Click here to obtain a Health Benefits Registration Form (SF2809) and return it to your employing office after you have completed it. They will fill out the rest of the form and send it to us;
  • Enroll through the electronic Employee Express if your agency has that capability. OPM will forward us the information; or
  • Enroll through the one of the Department of Defense's (DoD) automated systems. DoD will forward us your enrollment information.

Annuitants - To enroll in the FSBP as a Foreign Service Annuitant (Note - you cannot enroll in the Plan as a new enrollee unless you are a Foreign Service Annuitant. Civil Service Annuitants cannot enroll in the Plan as new enrollees.):

  • Contact the Foreign Service Retirement System through the Retirement Division at the Department of State.

Once we receive your enrollment information we will mail you a "Welcome Packet" indicating that we have processed your enrollment in the Plan. The “Welcome Packet” contains important information about your benefits and how to file claims for them. You should review it carefully when you receive it. The Plan's underwriter, Coventry, will mail you a combined FSBP/Medco ID Card under separate cover. This card serves as your FSBP Health ID and your Prescription Card.
