That’s why the FSBP makes it easy: take the Simple Steps outlined here and we’ll give you rewards worth $250 (you must be over 18 years of age to receive the incentives). 

Wellness Incentive Fund Account credits help reimburse you for Eligible Medical Expenses like your deductible, coinsurance, and copayments, as defined by IRS Code Section 213(d). You can obtain each reward once per calendar year.

You must complete the Health Risk Assessment and a Routine Physical Examination (Step 1) to be eligible to complete a Biometric Screening (Step 2).

Members identified with certain medical conditions may be eligible to receive additional rewards by completing Healthy Actions.

Completing your HRA annually is an important first step to guiding your personal health goals.

Simple Step 1: Take a Health Risk Assessment and have your Routine Physical Examination

The Health Risk Assessment

Reward: $75 Wellness Incentive Fund Credit

A Health Risk Assessment (HRA) is like an online interview. It asks questions like those asked during an annual checkup. You share information about your health habits and history. Then, you receive a personalized health summary to help you understand your health needs.

Take your Health Risk Assessment

1. Go to "Helpful Links", then to “Aetna Navigator®” and log on or register
2. Look for the “Stay Healthy” bar on the bottom right of the page and Select ''Discover a Healthier You.''
3. You are taken to a page that says ''For Your Information.'' Select Continue.
4. The next page will appear ''Wellness isn't far away!'' Click ''My Rewards'' to get started.
5. The next page will appear ''Wellness Rewards.'' At the bottom, select ''Work On It'' to start your Health Assessment.
6. Or, call 855-406-5122 to complete the HRA by phone.

Obtain a physical examination

An annual checkup helps detect health conditions early. When you complete this routine preventive visit with an in-network provider, there is no cost to you.

Step 1 (both parts) must be completed by December 1 of the calendar year to earn the $75 reward.

Simple Step 2: Complete a Biometric Screening

The Biometric Screening

Reward: $75 Wellness Incentive Fund Credit

A Biometric Screening includes measurements of physical characteristics such as height, weight, body mass index, blood pressure, blood cholesterol, blood glucose, and aerobic fitness. It serves as a benchmark and can help evaluate changes in health status over time. Obtain a Biometric Screening at a Quest Diagnostics Patient Service Center (PSC), or from your physician. After completing your screening, you will receive your results and a personalized action plan to help you better understand your health risks.

To earn the $75 reward, you must complete a Biometric Screening results and pass 3 out of 5 metabolic syndrome criteria – HDL Cholesterol, Triglycerides, Blood pressure, Waist Circumference, and Glucose. If you do not pass 3 out of 5 metabolic syndrome criteria, you can still receive a $75 Wellness Credit by completing one of these wellness programs– Digital Coaching, Mediterranean Wellness, Health Coaching, Virtual Lifestyle Management, or Dietary and Nutritional Counseling.

Healthy Action(s)

Reward: Up to $100 Wellness Incentive Fund Credit (You can earn only two $50 Healthy Action rewards per calendar year)

Members identified with the following conditions may be eligible to complete two out of the four Healthy Actions outlined below and earn $50 for each one completed to be deposited into a Wellness Incentive Fund Account.

Healthy Actions (with wellness programs, if applicable) must be completed by December 1 of the calendar year.

Your provider must document two controlled blood pressure readings (below 140/90) – on separate calendar year visits. If you are unable to meet this goal, you can earn a reward by completing one of the following wellness programs: Mediterranean Wellness, Health Coaching, or Virtual Lifestyle Management.

Your provider must document test results showing: a 5% weight reduction, at least a 10% reduction in triglycerides, or a 5% HDL increase. If you are unable to meet this goal, you can earn a reward by completing one of the following wellness programs: Mediterranean Wellness, Virtual Lifestyle Management, or Dietary and Nutritional counseling.

Your provider must document laboratory results and your HbA1c must be less than 8%. If you are unable to meet this goal, you can earn a reward by completing one of the following wellness programs: Virtual Lifestyle Management, Mediterranean Wellness, Dietary and Nutritional counseling, Diabetic Education, or Livongo Remote Diabetes Monitoring.

If you are pregnant, your provider must submit documentation of a prenatal care visit during the first trimester. The documentation submitted must include a copy of the prenatal care medical record of Obstetric Panel testing from your provider for you to earn the incentive.


Because we offer a wide variety of programs, in addition to our generous benefits, sometimes the condition for which a member needs help gets lost. The chart below focuses on possible member conditions and starts on the left column with the condition you may have. Then, we identify the program(s) or Healthy Action(s) available to achieve wellness and to maintain your health.

The first step to wellness is to obtain your routine physical and screenings. If you are well, great! However, if you do have a condition you need help with, we hope our Wellness Chart guides you to other programs or Healthy Actions from which you might benefit. And, don’t forget, when you take part in a Healthy Action, eligible members can earn a $50 Wellness Incentive Reward.

The levels of intensity are: Basic, which is generally an online interaction or Personalized, which generally involves one-on-one interaction (either online or telephonic) to discuss individual concerns as it is a more intensive process. Livongo-remote diabetes monitoring program

Livongo is a remote diabetes monitoring program powered by Livongo Health that empowers those with diabetes to live a better life. Livongo provides personalized support through a cellular enabled meter, mobile app, and personalized interventions to help you make better decisions about diabetes management. Candidates will be contacted and asked to participate in the program based on claims data. Participation is voluntary. The participant and his/her physician or health care professional remain in charge of the participant’s treatment plan. The program provides:

  • Unlimited blood glucose test strips and lancets
  • Strip reordering right from your meter
  • In Touch® glucose meter that tracks strip usage and prompts members to reorder supplies
  • Real-time interventions by Certified Diabetes Educators for members with dangerous (high and/or low) blood sugar levels

Mediterranean Wellness Program

The Mediterranean diet is known to be one of the healthiest diets in the world. For those who want to see gains in overall well-being and appreciate a preplanned structure, try this 8-week interactive online program that includes access to a registered dietician. You can receive up to 100% reimbursement for the Mediterranean Wellness Program once you complete at least 80% of the Program. Once you complete at least 80% of the Program, the Plan will automatically reimburse you for the cost of the program under the Plan’s Nutritional counseling benefit. 24-Hour Nurse Advice & Translation Lines 

FSBP offers two free 24-hour hotlines to assist our members in real time.

The Nurse Advice Line provides members with a live registered nurse to advise them about urgent issues and answer health-related questions. The Nurse Advice Line is 855-482-5750 or 705-834-6782. You can also engage a registered nurse in an online chat via Aetna Navigator. Once you log on the site, select "Wellness Tools" and then "Nurse Advice Line." In addition, you have access to the AudioHealth Library within Aetna Navigator

The Translation Line provides telephonic language translation services for our overseas members. This service provides direct translation for our members utilizing urgent or emergency services while outside of the U.S. The Translation Line is 855-482-5750 or 705-834-6782.

Pre-Diabetic Alert Program

The Pre-Diabetic Alert Program focuses on providing members with the tools they need to live healthier lives when facing a chronic illness – diabetes – before it happens. We proactively contact those members who are at risk for developing diabetes to invite them to participate in this program. Able To - on-line treatment support program

AbleTo is a web-based video conferencing personalized 8-week treatment support program designed to address the unique emotional and behavioral health needs of individuals learning to live with conditions like heart disease, type 2 diabetes, chronic pain or life events such as losing a loved one or having a baby. Members work with the same therapist and coach each week to set reasonable goals toward healthier lifestyle changes.

There are several ways we identify members who may benefit from the AbleTo support such as:

  • Your nurses or clinicians may refer you to AbleTo as they work directly with you and can refer you if it is determined that you can benefit from AbleTo support.
  • If identified, an Engagement Specialist from AbleTo will contact you to introduce the treatment option.

Cleveland Clinic 

Looking for accurate health information from a trusted source? Use the Cleveland Clinic for free, trustworthy medical information. While at their site, you can sign up for free e-newsletters, physician-hosted online chats, or look up specific health topics.

Using the Cleveland Clinic website, including participating in the physician-hosted online chats, for health information is not part of the FEHB contract or premium, so you cannot file an FEHB disputed claim about it or the chats they hold. 

Overview

At the Foreign Service Benefit Plan (FSBP), Caring for Your Health Worldwide is more than just a slogan – it is the driving force behind everything we do. That’s why the FSBP makes it easy: take the Simple Steps outlined here and we’ll give you rewards worth $250 (you must be over 18 years of age to receive the incentives). 

Wellness Incentive Fund Account credits help reimburse you for Eligible Medical Expenses like your deductible, coinsurance, and copayments, as defined by IRS Code Section 213(d). You can obtain each reward once per calendar year.

You must complete the Health Risk Assessment and a Routine Physical Examination (Step 1) to be eligible to complete a Biometric Screening (Step 2).

Members identified with certain medical conditions may be eligible to receive additional rewards by completing Healthy Actions.

Completing your HRA annually is an important first step to guiding your personal health goals.

Simple Step 1: Take a Health Risk Assessment and have your Routine Physical Examination

The Health Risk Assessment

Reward: $75 Wellness Incentive Fund Credit

A Health Risk Assessment (HRA) is like an online interview. It asks questions like those asked during an annual checkup. You share information about your health habits and history. Then, you receive a personalized health summary to help you understand your health needs.

Take your Health Risk Assessment

1. Go to "Helpful Links", then to “Aetna Navigator®” and log on or register
2. Look for the “Stay Healthy” bar on the bottom right of the page and Select ''Discover a Healthier You.''
3. You are taken to a page that says ''For Your Information.'' Select Continue.
4. The next page will appear ''Wellness isn't far away!'' Click ''My Rewards'' to get started.
5. The next page will appear ''Wellness Rewards.'' At the bottom, select ''Work On It'' to start your Health Assessment.
6. Or, call 855-406-5122 to complete the HRA by phone.

Obtain a physical examination

An annual checkup helps detect health conditions early. When you complete this routine preventive visit with an in-network provider, there is no cost to you.

Step 1 (both parts) must be completed by December 1 of the calendar year to earn the $75 reward.

Simple Step 2: Complete a Biometric Screening

The Biometric Screening

Reward: $75 Wellness Incentive Fund Credit

A Biometric Screening includes measurements of physical characteristics such as height, weight, body mass index, blood pressure, blood cholesterol, blood glucose, and aerobic fitness. It serves as a benchmark and can help evaluate changes in health status over time. Obtain a Biometric Screening at a Quest Diagnostics Patient Service Center (PSC), or from your physician. After completing your screening, you will receive your results and a personalized action plan to help you better understand your health risks.

To earn the $75 reward, you must complete a Biometric Screening results and pass 3 out of 5 metabolic syndrome criteria – HDL Cholesterol, Triglycerides, Blood pressure, Waist Circumference, and Glucose. If you do not pass 3 out of 5 metabolic syndrome criteria, you can still receive a $75 Wellness Credit by completing one of these wellness programs– Digital Coaching, Mediterranean Wellness, Health Coaching, Virtual Lifestyle Management, or Dietary and Nutritional Counseling.

Healthy Action(s)

Reward: Up to $100 Wellness Incentive Fund Credit (You can earn only two $50 Healthy Action rewards per calendar year)

Members identified with the following conditions may be eligible to complete two out of the four Healthy Actions outlined below and earn $50 for each one completed to be deposited into a Wellness Incentive Fund Account.

Healthy Actions (with wellness programs, if applicable) must be completed by December 1 of the calendar year.

Your provider must document two controlled blood pressure readings (below 140/90) – on separate calendar year visits. If you are unable to meet this goal, you can earn a reward by completing one of the following wellness programs: Mediterranean Wellness, Health Coaching, or Virtual Lifestyle Management.

Your provider must document test results showing: a 5% weight reduction, at least a 10% reduction in triglycerides, or a 5% HDL increase. If you are unable to meet this goal, you can earn a reward by completing one of the following wellness programs: Mediterranean Wellness, Virtual Lifestyle Management, or Dietary and Nutritional counseling.

Your provider must document laboratory results and your HbA1c must be less than 8%. If you are unable to meet this goal, you can earn a reward by completing one of the following wellness programs: Virtual Lifestyle Management, Mediterranean Wellness, Dietary and Nutritional counseling, Diabetic Education, or Livongo Remote Diabetes Monitoring.

If you are pregnant, your provider must submit documentation of a prenatal care visit during the first trimester. The documentation submitted must include a copy of the prenatal care medical record of Obstetric Panel testing from your provider for you to earn the incentive.

Wellness Chart

Because we offer a wide variety of programs, in addition to our generous benefits, sometimes the condition for which a member needs help gets lost. The chart below focuses on possible member conditions and starts on the left column with the condition you may have. Then, we identify the program(s) or Healthy Action(s) available to achieve wellness and to maintain your health.

The first step to wellness is to obtain your routine physical and screenings. If you are well, great! However, if you do have a condition you need help with, we hope our Wellness Chart guides you to other programs or Healthy Actions from which you might benefit. And, don’t forget, when you take part in a Healthy Action, eligible members can earn a $50 Wellness Incentive Reward.

The levels of intensity are: Basic, which is generally an online interaction or Personalized, which generally involves one-on-one interaction (either online or telephonic) to discuss individual concerns as it is a more intensive process. Livongo

Livongo-remote diabetes monitoring program

Livongo is a remote diabetes monitoring program powered by Livongo Health that empowers those with diabetes to live a better life. Livongo provides personalized support through a cellular enabled meter, mobile app, and personalized interventions to help you make better decisions about diabetes management. Candidates will be contacted and asked to participate in the program based on claims data. Participation is voluntary. The participant and his/her physician or health care professional remain in charge of the participant’s treatment plan. The program provides:

  • Unlimited blood glucose test strips and lancets
  • Strip reordering right from your meter
  • In Touch® glucose meter that tracks strip usage and prompts members to reorder supplies
  • Real-time interventions by Certified Diabetes Educators for members with dangerous (high and/or low) blood sugar levels
  • Personalized tips with each blood glucose check
  • Transfer of your data directly to your provider via the meter
  • Automatic uploads (no more paper logbooks)

Virtual Lifestyle Management Program (VLM)

Perhaps an online support program is appealing, and your goal is to lose weight and keep it off. Try this year-long program focused on nutrition and weight management. Mediterranean Wellness Program

The Mediterranean diet is known to be one of the healthiest diets in the world. For those who want to see gains in overall well-being and appreciate a preplanned structure, try this 8-week interactive online program that includes access to a registered dietician. You can receive up to 100% reimbursement for the Mediterranean Wellness Program once you complete at least 80% of the Program. Once you complete at least 80% of the Program, the Plan will automatically reimburse you for the cost of the program under the Plan’s Nutritional counseling benefit. This program consists of regular phone calls with a credentialed health coach. The trained health coaches at TrestleTree have helped FSBP members get into regular exercise routines, deal with stressors in their lives, create a plan for tobacco cessation, and lose weight. 24-Hour Nurse Advice & Translation Lines 

FSBP offers two free 24-hour hotlines to assist our members in real time.

The Nurse Advice Line provides members with a live registered nurse to advise them about urgent issues and answer health-related questions. The Nurse Advice Line is 855-482-5750 or 705-834-6782. You can also engage a registered nurse in an online chat via Aetna Navigator. Once you log on the site, select "Wellness Tools" and then "Nurse Advice Line." In addition, you have access to the AudioHealth Library within Aetna Navigator

The Translation Line provides telephonic language translation services for our overseas members. This service provides direct translation for our members utilizing urgent or emergency services while outside of the U.S. The Translation Line is 855-482-5750 or 705-834-6782.

Pre-Diabetic Alert Program

The Pre-Diabetic Alert Program focuses on providing members with the tools they need to live healthier lives when facing a chronic illness – diabetes – before it happens. We proactively contact those members who are at risk for developing diabetes to invite them to participate in this program. The goal is to inform members of all the tools at their disposal to stay as healthy as possible. Please call the Plan at 800-593-2354 if you would like more information about this program. Able To - on-line treatment support program

AbleTo is a web-based video conferencing personalized 8-week treatment support program designed to address the unique emotional and behavioral health needs of individuals learning to live with conditions like heart disease, type 2 diabetes, chronic pain or life events such as losing a loved one or having a baby. Members work with the same therapist and coach each week to set reasonable goals toward healthier lifestyle changes.

There are several ways we identify members who may benefit from the AbleTo support such as:

  • Your nurses or clinicians may refer you to AbleTo as they work directly with you and can refer you if it is determined that you can benefit from AbleTo support.
  • If identified, an Engagement Specialist from AbleTo will contact you to introduce the treatment option.
  • If you feel you would benefit from this program, would like more information, or would like to enroll in this program please call 866-287-1802 or visit AbleTo’s website at www.AbleTo.com/enroll.

Cleveland Clinic 

Looking for accurate health information from a trusted source? Use the Cleveland Clinic for free, trustworthy medical information. While at their site, you can sign up for free e-newsletters, physician-hosted online chats, or look up specific health topics.

Using the Cleveland Clinic website, including participating in the physician-hosted online chats, for health information is not part of the FEHB contract or premium, so you cannot file an FEHB disputed claim about it or the chats they hold.

What is the calendar year deductible?
  • For Self Only (401) enrollment, the deductible is $300 for in-network providers (including Guam) and providers outside the U.S., and $400 for out-of-network providers (including Guam).
  • For Self Plus One (403) enrollment, the deductible is $600 for in-network providers (including Guam) and providers outside the U.S., and $800 for out-of-network providers (including Guam).
  • Under a Self and Family (402) enrollment, the deductible is $600 for in-network providers (including Guam) and providers outside the U.S., and $800 for out-of-network providers (including Guam).
What's the difference between Aetna Navigator and the Member Portal?

Aetna Navigator is a system that provides members with access to information and tools regarding their health. Every enrollee and dependents ages 14 and older have their own separate login credentials for this site.

Many FSBP programs are housed in your Aetna Navigator account, including signing up for wellness incentives, seeing your Health Risk Assessment results, opting into electronic funds transfer (EFT), learning more about the quality and cost of your health care, and viewing your Explanations of Benefits (EOBs).


The Member Portal is a custom system for AFSPA members. It can be used to upload claims, update addresses, and enroll in and manage certain Ancillary Insurance Programs. The Member Portal is only open to the AFSPA member, not dependents. We recommend having a Member Portal account in order to allow for the fastest processing of claims.

*You can still access your claims for December 31, 2017, and earlier at My Online Services (MOS). You may want to download your claims and EOBs before the site is taken down at the end of 2019.

Does the Plan have a catastrophic protection out-of-pocket maximum for deductibles, coinsurance, and copayments?

For those benefits where copayments, coinsurance or deductibles apply, we pay 100% of the Plan allowance for the rest of the calendar year after your expenses total to:

  • For Self Only enrollment $5,000 and for Self Plus One or Self and Family enrollment $7,000 for in-network providers (including Guam) and providers outside the 50 United States and when you use the Plan’s network retail pharmacy through Express Scripts (ESI), or home delivery (mail order) through the Express Scripts PharmacySM, or purchase prescriptions outside the 50 United States from a retail pharmacy or Military Treatment Facility (including Guam);
  • For Self Only enrollment $7,000 and for Self Plus One or Self and Family enrollment $9,000 for in- and out-of-network providers combined (including Guam) and when you use the Plan’s network retail pharmacy through Express Scripts or home delivery (mail order) through the Express Scripts PharmacySM or purchase prescriptions outside the 50 United States from a retail pharmacy or Military Treatment Facility (including Guam).

For Self Plus One and Self and Family enrollments, once any individual family member reaches the Self Only catastrophic protection out-of-pocket maximum during the calendar year, that member’s claims will no longer be subject to associated cost-sharing amounts for the rest of the year. All other family members will be required to meet the balance of the catastrophic protection out-of-pocket maximum.

Any expenses incurred that apply toward the catastrophic out-of-pocket maximum for in-network or out-of-network apply toward both in and out-of-network limits.

This catastrophic protection out-of-pocket maximum is combined for medical/surgical, mental health/substance misuse disorder, and pharmacy. There are some expenses that do not fall under this provision; see your FSBP FSBP Brochure, Section 4, Your Costs for Covered Services.
