AFSPA MEMBER PORTAL

The Member Portal is a secure gateway designed for our members. This feature allows Private Messages to be sent to our members to maintain compliance with HIPAA, SOX and other Federal regulations. AFSPA is required to protect its members’ Personal Identifiable Information (PII), Protected Health Information (PHI) and all financial information. When our staff’s reply to your e-mail contains sensitive information or PHI, you will receive an e-mail notification from AFSPA, at the e-mail address on file, notifying you to log in to the Member Portal to read the Private Message. To submit a claim (health only) or enrollment/application in a confidential manner, simply click on the appropriate insurance plan section and follow the on screen prompts.

To log in to the Member Portal, you must be enrolled in one of our insurance programs and be the primary member, not a spouse or dependent. You may update your contact information, including preferred communication method with AFSPA (e-mail or USPS mail) at any time.

  2. Click on “First time user?”
  3. Enter the e-mail address on file with AFSPA.

Disease Management Programs & Wellness Incentives

The Plan offers Wellness Incentives to help you maintain good health when you participate in one of the following Plan’s Disease Management Programs:

  • Diabetes Disease Management Program – You must obtain the following services:
    • Annual physician visit
    • LDL and Micro albumin tests
    • Hemoglobin A1C blood test
    • Dilated retinal eye exam
  • Coronary Artery Disease Management Program – You must meet obtain the following services:
    • Annual physician visit
    • Prescription for ACE Inhibitor or ARB per your physician’s orders
    • Prescription for Beta Blocker per your physician’s orders
    • LDL test
  • Asthma Disease Management Program – You must obtain the following services:
    • Annual physician visit
    • Appropriate medication (or submit a letter from your physician stating no medication is required)
    • Spirometry test within the past 24 months

The Plan offers Wellness Incentives to help you maintain good health when you are diagnosed with Diabetes, Coronary Artery Disease or Asthma. Through this program, you can earn $50 per condition to be deposited in a Wellness Incentive Coventry Fund Account to reimburse you for certain unreimbursed medical expenses (“Eligible Medical Expenses”). Eligible Medical Expenses, as defined by Internal Revenue Code Section 213(d), include your deductible, coinsurance and copayments (e.g., prescription drug copayments) incurred by you or your covered dependents. You and your dependents’ medical claims and prescription claims submitted for non-network retail pharmacies outside the 50 United States will automatically transfer to the Wellness Incentive Coventry Fund after processing. Reimbursement of your deductible, coinsurance and non-prescription drug copayments will be sent to you if there are funds available. Other expenses, like dental, vision, and prescriptions purchased through the Plan’s retail pharmacy network or home delivery programs will not be automatically reimbursed. You will need to submit appropriate documentation (receipts, etc.) with a claim form that can be found on My Online ServicesSM (MOS).

  • Log on to MOS
  • Select “Benefits”
  • Select “Manage My HRA/FSA or Coventry Fund”

Any unused funds in the Wellness Incentive Coventry Fund at the end of the calendar year will remain in the Wellness Incentive Coventry Fund Account for Eligible Medical Expenses in the next Plan year as long as you remain enrolled in the Plan.

To monitor the availability of funds in your Wellness Incentive Coventry Fund Account, visit the Plan’s Web site ( www.AFSPA.org/FSBP) and click on “My Online Services” (MOS).
