Overseas Health Care Facilities with Direct Billing Arrangements Secure Claim Submission Form
This form is intended solely for healthcare providers and U.S. Embassies and Consulates that have direct billing arrangements with AFSPA. It is for submitting bills and invoices directly to AFSPA for services provided to members of the Foreign Service Benefit Plan.
Note: When submitting your claims, please use the appropriate secure form and ensure that you include all the necessary information for processing. To confirm that you have everything required, please review the guidelines here before submitting your claim.
Additionally, please combine the member’s bill or invoice along with any other relevant documents into a single file to prevent delays in claim processing. The maximum file size allowed is 50 MB.
