Wellness Incentive Fund Account credits help reimburse you for Eligible Medical Expenses like your deductible, coinsurance, and copayments, as defined by IRS Code Section 213(d). You can obtain each reward once per calendar year.

You must complete the Health Risk Assessment (Step 1) to be eligible to complete a Biometric Screening (Step 2).

But you can perform a Healthy Action at any time, independent of the other two steps!

Completing your HRA annually is an important first step to guiding your personal health goals.

Simple Step 1: Take a Health Risk Assessment

A Health Risk Assessment (HRA) is like an online interview. It asks questions like those asked during an annual checkup. You share information about your health habits and history. Ten, you receive a personalized health summary to help you understand your health needs.

3. You are taken to a page that says ''For Your Information.'' Select Continue.

5. The next page will appear ''Wellness Rewards.'' At the bottom, select ''Work On It'' to start your Health Assessment.

Simple Step 2: Complete a Biometric Screening

The Biometric Screening

Reward: $100 Wellness Incentive Fund Credit

A Biometric Screening includes measurements of physical characteristics such as height, weight, body mass index, blood pressure, blood cholesterol, blood glucose, and aerobic fitness. It serves as a benchmark and can help evaluate changes in health status over time. Obtain a Biometric Screening at a Quest Diagnostics Patient Service Center (PSC), or from your physician. After completing Step 2, you will receive your results and a personalized action plan to help you better understand your health risks.

FSBP offers a comprehensive variety of wellness programs. Members can participate in as many programs as make sense for their lives. It’s hard to make real changes that impact your health, so completing just one of these actions is all that’s needed to claim your $50 wellness credit.

Healthy Actions for 2018 include;

The Plan will reach out to you if you are identified through claims data as having high blood pressure and will provide you with a form for your provider to complete. On the form, your provider must document two (2) controlled blood pressure readings below 140/90 on separate visits during the current calendar year for you to earn the incentive. The completed form must be submitted to the Plan by December 1st of the calendar year. To earn the incentive, documentation should be mailed to: Aetna, C/O FEHB QM, 4400 NW Loop 410, Suite 101, San Antonio, TX 78229.

The Plan will reach out to you if you are identified through claims data as having diabetes and ask you to have your provider submit your A1c laboratory results. Your A1c laboratory results must be less than 8% during the calendar year for you to earn the incentive. Your A1c laboratory results must be submitted to the Plan by December 1st of the calendar year. If you are pregnant, your provider must submit documentation of a prenatal care visit during the first trimester. The documentation submitted must include a copy of the prenatal care medical record of Obstetric Panel testing from your attending provider for you to earn the incentive. The documentation must be submitted to the Plan by December 1st of the calendar year. To earn the incentive, documentation should be mailed to: Aetna, C/O FEHB QM, 4400 NW Loop 410, Suite 101, San Antonio, TX 78229.

It’s always good to start at the beginning: call your primary care or family physician today to schedule a physical exam (or a routine gynecological exam). This is an especially important step if you took the Biometric Screening at a Quest Diagnostics Center. Bring your new knowledge back to a doctor you want to work with, and ask them: What is the next healthy action I should take? Don’t forget: When you have your routine physical exam performed by an in-network provider or a provider outside the 50 states, the Plan pays for covered services at 100% Plan allowance – with no cost-sharing! If you’re a tobacco user, you know both how bad it is for your health and how hard it is to quit. The FSBP is here for you. The Mediterranean diet is known to be one of the healthiest diets in the world.* For those who want to see gains in overall well-being and appreciate a preplanned structure, try this 8-week interactive online program that includes access to a registered dietician. You can receive up to 100% reimbursement for the Mediterranean Wellness Program once you complete at least 80% of the Program. Once you complete at least 80% of the Program, the Plan will automatically reimburse you for the cost of the program under the Plan’s Nutritional counseling benefit. In addition to our standard support for members going through cancer treatment, the TherapEase Cuisine program helps cancer patients answer the question “What should I be eating?” Provided in an online format by the Express Scripts Pharmacy (the Plan’s Pharmacy Benefit Manager), it follows the American Dietetic Association’s guidelines for cancer nutrition. Because we offer a wide variety of programs, in addition to our generous benefits, sometimes the condition for which a member needs help gets lost. The chart below focuses on possible member conditions and starts on the left column with the condition you may have. Then, we identify the program(s) or Healthy Action(s) available to achieve wellness and to maintain your health.

The first step to wellness is to obtain your routine physical and screenings. If you are well, great! By obtaining your routine physical exam, you have completed one Healthy Action already. However, if you do have a condition you need help with, we hope our Wellness Chart guides you to other programs or Healthy Actions from which you might benefit. And, don’t forget, when you take part in a Healthy Action, you earn a $50 Wellness Incentive Reward. You can find other Healthy Actions on page 88 of your 2018 FSBP Brochure.

The levels of intensity are: Basic, which is generally an online interaction or Personalized, which generally involves one-on-one interaction (either online or telephonic) to discuss individual concerns as it is a more intensive process. Our wellness programs include additional options outside of coaching-style healthy actions. As your health insurance plan, the FSBP works closely with many partners to provide you with professional services to keep you healthy. 

24-Hour Nurse Advice & Translation Lines 

FSBP offers two free 24-hour hotlines to assist our members in real time.

The Nurse Advice Line provides members with a live registered nurse to advise them about urgent issues and answer health-related questions. The Nurse Advice Line is 855-482-5750 or 705-834-6782. You can also engage a registered nurse in an online chat via Aetna Navigator. Once you log on the site, select "Wellness Tools" and then "Nurse Advice Line." In addition, you have access to the AudioHealth Library within Aetna Navigator

The Translation Line provides telephonic language translation services for our overseas members. This service provides direct translation for our members utilizing urgent or emergency services while outside of the U.S. The Translation Line is 855-482-5750 or 705-834-6782.

KidsHealth

KidsHealth is a complementary and comprehensive library of childhood health information featuring thousands of medically reviewed articles, animations, and age-appropriate news for three distinct audiences: parents, kids, and teens. Log on to Aetna Navigator to access KidsHealth.

Use this online library to access the tools you need as a parent to teach your children how to lead healthier lives. The special kids' sections include fun and age-appropriate information, recipes, quizzes, and movies. The site includes information about dealing with emotions, stress, and school. Case Management Program

FSBP offers free and voluntary case management to all enrollees. Case management assists you, your dependents, and your health care providers to address acute, complex, and/or long-term medical needs. Case management services include nurse support; education about disease, illness, and injury; and proactive medical management that can help you lead a healthier life. Livongo-remote diabetes monitoring program

Livongo is a remote diabetes monitoring program powered by Livongo Health that empowers those with diabetes to live a better life. Livongo provides personalized support through a cellular enabled meter, mobile app, and personalized interventions to help you make better decisions about diabetes management. Candidates will be contacted and asked to participate in the program. Participation is voluntary. The participant and his/her physician or health care professional remain in charge of the participant’s treatment plan. The program provides:

  • Unlimited blood glucose test strips and lancets
  • Strip reordering right from your meter
  • In Touch® glucose meter that tracks strip usage and prompts members with supplies
  • Real-time interventions by Certified Diabetes Educators for members with dangerous (high and/or low) blood sugar levels

Using the Cleveland Clinic website, including participating in the physician-hosted online chats, for health information is not part of the FEHB contract or premium, so you cannot file an FEHB disputed claim about it or the chats they hold. 

 

Dream Program 

With the Dream Program you will be walked through the steps to help improve your sleep, increase your daytime energy, and enhance your quality of life. You will be introduced to certain principles of cognitive behavioral therapy (CBT). There will be a consultation followed by a five-week program that includes coaching and an online sleep tracker. Able To - on-line treatment support program

AbleTo is a web-based video conferencing personalized 8-week treatment support program designed to address the unique emotional and behavioral health needs of individuals learning to live with conditions like heart disease, type 2 diabetes, chronic pain or life events such as losing a loved one or having a baby. Members work with the same therapist and coach each week to set reasonable goals toward healthier lifestyle changes.

There are several ways we identify members who may benefit from the AbleTo support such as:

  • Your nurses or clinicians may refer you to AbleTo as they work directly with you and can refer you if it is determined that you can benefit from AbleTo support.
  • If identified, an Engagement Specialist from AbleTo will contact you to introduce the treatment option.

The Advanced Illness (AI) Program is designed to improve the quality of life through health condition management and to reduce costs for members with advanced illness, including those facing imminent end-of life decisions. It provides tools and information to encourage advance planning for the kind of issues often associated with an advanced illness, such as living wills, advance directives, and tips on how to begin conversations about these issues with loved ones. This program is designed to provide quality of life improvement through health condition management and to reduce costs for members at the end of life through timely member and caregiver education. It encourages better use of community-based services and resources, systemic palliative care integration and enhanced hospice utilization and retention. This program is a voluntary program provided to you and your dependents at no additional cost. Overview

At the Foreign Service Benefit Plan (FSBP), Caring for Your Health Worldwide is more than just a slogan – it is the driving force behind everything we do. Wellness Incentive Fund Account credits help reimburse you for Eligible Medical Expenses like your deductible, coinsurance, and copayments, as defined by IRS Code Section 213(d). You can obtain each reward once per calendar year.

You must complete the Health Risk Assessment (Step 1) to be eligible to complete a Biometric Screening (Step 2).

But you can perform a Healthy Action at any time, independent of the other two steps!

A Health Risk Assessment (HRA) is like an online interview. It asks questions like those asked during an annual checkup. You share information about your health habits and history. Ten, you receive a personalized health summary to help you understand your health needs.

A Biometric Screening includes measurements of physical characteristics such as height, weight, body mass index, blood pressure, blood cholesterol, blood glucose, and aerobic fitness. It serves as a benchmark and can help evaluate changes in health status over time. Obtain a Biometric Screening at a Quest Diagnostics Patient Service Center (PSC), or from your physician. After completing Step 2, you will receive your results and a personalized action plan to help you better understand your health risks.

FSBP offers a comprehensive variety of wellness programs. Members can participate in as many programs as make sense for their lives. It’s hard to make real changes that impact your health, so completing just one of these actions is all that’s needed to claim your $50 wellness credit.

Healthy Actions for 2018 include;

The Plan will reach out to you if you are identified through claims data as having high blood pressure and will provide you with a form for your provider to complete. On the form, your provider must document two (2) controlled blood pressure readings below 140/90 on separate visits during the current calendar year for you to earn the incentive. The completed form must be submitted to the Plan by December 1st of the calendar year. To earn the incentive, documentation should be mailed to: Aetna, C/O FEHB QM, 4400 NW Loop 410, Suite 101, San Antonio, TX 78229.

The Plan will reach out to you if you are identified through claims data as having diabetes and ask you to have your provider submit your A1c laboratory results. Your A1c laboratory results must be less than 8% during the calendar year for you to earn the incentive. Your A1c laboratory results must be submitted to the Plan by December 1st of the calendar year. To earn the incentive, documentation should be mailed to: Aetna, C/O FEHB QM, 4400 NW Loop 410, Suite 101, San Antonio, TX 78229.

If you are pregnant, your provider must submit documentation of a prenatal care visit during the first trimester. The documentation submitted must include a copy of the prenatal care medical record of Obstetric Panel testing from your attending provider for you to earn the incentive. The documentation must be submitted to the Plan by December 1st of the calendar year. To earn the incentive, documentation should be mailed to: Aetna, C/O FEHB QM, 4400 NW Loop 410, Suite 101, San Antonio, TX 78229.

It’s always good to start at the beginning: call your primary care or family physician today to schedule a physical exam (or a routine gynecological exam). This is an especially important step if you took the Biometric Screening at a Quest Diagnostics Center. Bring your new knowledge back to a doctor you want to work with, and ask them: What is the next healthy action I should take? Don’t forget: When you have your routine physical exam performed by an in-network provider or a provider outside the 50 states, the Plan pays for covered services at 100% Plan allowance – with no cost-sharing! If you’re a tobacco user, you know both how bad it is for your health and how hard it is to quit. The FSBP is here for you. The Mediterranean diet is known to be one of the healthiest diets in the world.* For those who want to see gains in overall well-being and appreciate a preplanned structure, try this 8-week interactive online program that includes access to a registered dietician. You can receive up to 100% reimbursement for the Mediterranean Wellness Program once you complete at least 80% of the Program. Once you complete at least 80% of the Program, the Plan will automatically reimburse you for the cost of the program under the Plan’s Nutritional counseling benefit. Because we offer a wide variety of programs, in addition to our generous benefits, sometimes the condition for which a member needs help gets lost. The chart below focuses on possible member conditions and starts on the left column with the condition you may have. Then, we identify the program(s) or Healthy Action(s) available to achieve wellness and to maintain your health.

The first step to wellness is to obtain your routine physical and screenings. If you are well, great! By obtaining your routine physical exam, you have completed one Healthy Action already. However, if you do have a condition you need help with, we hope our Wellness Chart guides you to other programs or Healthy Actions from which you might benefit. Our wellness programs include additional options outside of coaching-style healthy actions. As your health insurance plan, the FSBP works closely with many partners to provide you with professional services to keep you healthy. 

24-Hour Nurse Advice & Translation Lines 

FSBP offers two free 24-hour hotlines to assist our members in real time.

The Nurse Advice Line provides members with a live registered nurse to advise them about urgent issues and answer health-related questions. The Nurse Advice Line is 855-482-5750 or 705-834-6782. You can also engage a registered nurse in an online chat via Aetna Navigator. Once you log on the site, select "Wellness Tools" and then "Nurse Advice Line." In addition, you have access to the AudioHealth Library within Aetna Navigator

The Translation Line provides telephonic language translation services for our overseas members. This service provides direct translation for our members utilizing urgent or emergency services while outside of the U.S. The Translation Line is 855-482-5750 or 705-834-6782.

KidsHealth

KidsHealth is a complementary and comprehensive library of childhood health information featuring thousands of medically reviewed articles, animations, and age-appropriate news for three distinct audiences: parents, kids, and teens. Log on to Aetna Navigator to access KidsHealth.

Use this online library to access the tools you need as a parent to teach your children how to lead healthier lives. The special kids' sections include fun and age-appropriate information, recipes, quizzes, and movies. The site includes information about dealing with emotions, stress, and school. Case Management Program

FSBP offers free and voluntary case management to all enrollees. Case management assists you, your dependents, and your health care providers to address acute, complex, and/or long-term medical needs. Case management services include nurse support; education about disease, illness, and injury; and proactive medical management that can help you lead a healthier life. Pre-Diabetic Alert Program

The Pre-Diabetic Alert Program focuses on providing members with the tools they need to live healthier lives when facing a chronic illness – diabetes – before it happens. We proactively contact those members who are at risk for developing diabetes to invite them to participate in this program. Livongo-remote diabetes monitoring program

Livongo is a remote diabetes monitoring program powered by Livongo Health that empowers those with diabetes to live a better life. Livongo provides personalized support through a cellular enabled meter, mobile app, and personalized interventions to help you make better decisions about diabetes management. Candidates will be contacted and asked to participate in the program. Participation is voluntary. The participant and his/her physician or health care professional remain in charge of the participant’s treatment plan. The program provides:

  • Unlimited blood glucose test strips and lancets
  • Strip reordering right from your meter
  • In Touch® glucose meter that tracks strip usage and prompts members with supplies
  • Real-time interventions by Certified Diabetes Educators for members with dangerous (high and/or low) blood sugar levels
  • Personalized tips with each blood glucose check
  • Transfer of your data directly to your provider via the meter
  • Automatic uploads (no more paper logbooks)

Looking for accurate health information from a trusted source? Use the Cleveland Clinic for free, trustworthy medical information. While at their site, you can sign up for free e-newsletters, physician-hosted online chats, or look up specific health topics.

Using the Cleveland Clinic website, including participating in the physician-hosted online chats, for health information is not part of the FEHB contract or premium, so you cannot file an FEHB disputed claim about it or the chats they hold. 

 

Dream Program 

With the Dream Program you will be walked through the steps to help improve your sleep, increase your daytime energy, and enhance your quality of life. You will be introduced to certain principles of cognitive behavioral therapy (CBT). There will be a consultation followed by a five-week program that includes coaching and an online sleep tracker. FSBP partners with myStrength to help our members access mental health support confidentially. myStrength is a unique online approach to putting mental wellness tools into the hands of FSBP members who need them. The program makes it easier for those facing some of the most common challenges – depression, anxiety, and substance abuse – to access real, evidence-based therapies, anywhere, anytime. 

All covered members ages 13 and older can sign up individually using the access code “FSBP.” New myStrength members first complete a brief Wellness Assessment and then are provided with modules specifically designed to help them deal with depression, anxiety, and substance abuse disorders.

AbleTo

Able To - on-line treatment support program

AbleTo is a web-based video conferencing personalized 8-week treatment support program designed to address the unique emotional and behavioral health needs of individuals learning to live with conditions like heart disease, type 2 diabetes, chronic pain or life events such as losing a loved one or having a baby. Members work with the same therapist and coach each week to set reasonable goals toward healthier lifestyle changes.

There are several ways we identify members who may benefit from the AbleTo support such as:

  • Your nurses or clinicians may refer you to AbleTo as they work directly with you and can refer you if it is determined that you can benefit from AbleTo support.
  • If identified, an Engagement Specialist from AbleTo will contact you to introduce the treatment option.

Advanced Illness Program

The Advanced Illness (AI) Program is designed to improve the quality of life through health condition management and to reduce costs for members with advanced illness, including those facing imminent end-of life decisions. It provides tools and information to encourage advance planning for the kind of issues often associated with an advanced illness, such as living wills, advance directives, and tips on how to begin conversations about these issues with loved ones. This program is designed to provide quality of life improvement through health condition management and to reduce costs for members at the end of life through timely member and caregiver education. It encourages better use of community-based services and resources, systemic palliative care integration and enhanced hospice utilization and retention. This program is a voluntary program provided to you and your dependents at no additional cost. What is the calendar year deductible?
  • For Self Only (401) enrollment, the deductible is $300 for in-network providers (including Guam) and providers outside the U.S., and $400 for out-of-network providers (including Guam).
  • For Self Plus One (403) enrollment, the deductible is $600 for in-network providers (including Guam) and providers outside the U.S., and $800 for out-of-network providers (including Guam).
  • Under a Self and Family (402) enrollment, the deductible is $600 for in-network providers (including Guam) and providers outside the U.S., and $800 for out-of-network providers (including Guam).
What's the difference between Aetna Navigator and the Member Portal?

Aetna Navigator is a system that provides members with access to information and tools regarding their health. Every enrollee and dependents ages 14 and older have their own separate login credentials for this site.

Many FSBP programs are housed in your Aetna Navigator account, including signing up for wellness incentives, seeing your Health Risk Assessment results, opting into electronic funds transfer (EFT), learning more about the quality and cost of your health care, and viewing your Explanations of Benefits (EOBs).


The Member Portal is a custom system for AFSPA members. It can be used to upload claims, update addresses, and enroll in and manage certain Ancillary Insurance Programs. The Member Portal is only open to the AFSPA member, not dependents. We recommend having a Member Portal account in order to allow for the fastest processing of claims.

*You can still access your claims for December 31, 2017, and earlier at My Online Services (MOS). You may want to download your claims and EOBs before the site is taken down at the end of 2018.

 

Does the Plan have a catastrophic protection out-of-pocket maximum for deductibles, coinsurance, and copayments?

For those benefits where copayments, coinsurance or deductibles apply, we pay 100% of the Plan allowance for the rest of the calendar year after your expenses total to:

  • For Self Only enrollment $5,000 and for Self Plus One or Self and Family enrollment $7,000 for in-network providers (including Guam) and providers outside the 50 United States and when you use the Plan’s network retail pharmacy through Express Scripts (ESI), or home delivery (mail order) through the Express Scripts PharmacySM, or purchase prescriptions outside the 50 United States from a retail pharmacy or Military Treatment Facility (including Guam);
  • For Self Only enrollment $7,000 and for Self Plus One or Self and Family enrollment $9,000 for in- and out-of-network providers combined (including Guam) and when you use the Plan’s network retail pharmacy through Express Scripts or home delivery (mail order) through the Express Scripts PharmacySM or purchase prescriptions outside the 50 United States from a retail pharmacy or Military Treatment Facility (including Guam).

For Self Plus One and Self and Family enrollments, once any individual family member reaches the Self Only catastrophic protection out-of-pocket maximum during the calendar year, that member’s claims will no longer be subject to associated cost-sharing amounts for the rest of the year. All other family members will be required to meet the balance of the catastrophic protection out-of-pocket maximum.

Any expenses incurred that apply toward the catastrophic out-of-pocket maximum for in-network or out-of-network apply toward both in and out-of-network limits.

This catastrophic protection out-of-pocket maximum is combined for medical/surgical, mental health/substance misuse disorder, and pharmacy. There are some expenses that do not fall under this provision; see your FSBP Brochure, Section 4, Your Costs for Covered Services.
