For 2026, the Foreign Service Benefit Plan introduced a new requirement for GLP-1 medications for weight loss under a diagnosis of obesity. This change led to numerous questions from members and potential members during Open Season. We compiled the most common questions asked during Open Season Webinars about GLP-1s into this blog as an FAQ guide.

What is a GLP-1 medication?

GLP-1 medications can help lower blood sugar levels and help with weight loss. According to the Cleveland Clinic, GLP-1 is a hormone produced by the small intestine. It helps with the following:

  • Triggers insulin to be released from your pancreas
  • Blocks your blood sugar from increasing
  • Slows digestion so that your body releases less sugar from the food you eat
  • Increases how full you feel after eating

GLP-1 medications are called GLP-1 agonists because they mimic the GLP-1 hormone in your body. GLP-1 agonists are extremely helpful for those with type 2 diabetes and obesity.

What is the new requirement?

FSBP will require members prescribed GLP-1 medications for weight loss under an obesity diagnosis to participate in the lifestyle management part of Encircle Rx. This includes using Teladoc Health’s Advanced Weight Management platform, which offers a free smart scale and an in-app program. Members must weigh in at least four times within a rolling 30-day window and complete four engagement activities within the app. Participation in this program is mandatory.

Do I have to participate in this program if I take GLP-1s for diabetes?

No

What conditions must I meet for GLP-1s to be covered?

GLP-1 drugs for weight loss will be covered* if you meet and your provider certifies certain conditions such as, but not limited to:

  • Body mass index (BMI) of 32 or greater when you began taking the drug; or
  • BMI of 27 or greater and documented diagnosis of at least two comorbidities when you began taking the drug; and
  • Patient has engaged in a trial of behavioral modification and dietary restriction for at least 3 months.

In addition to meeting these clinical requirements, you must enroll in Teladoc Health.

What counts as a comorbidity?

Some examples are hypertension, high cholesterol, and certain forms of cardiovascular disease (e.g, prior stroke or heart attack).

How do I enroll?

Enroll in the program by visiting https://www.teladochealth.com/livongo/benefits/fsbp or using the Teladoc Health app on your mobile device.* After enrolling, a smart scale will be sent to you.

When do I need to enroll by?

You must enroll before your first fill or refill of a GLP-1.

How do I meet my monthly requirements?

Weigh in at least four times using your smart scale. Engage with the program at least four times. Engagements can include coaching sessions with certified health coach, tracking your meals, reading an educational article, and more.

Do I need to get a new prior authorization in 2026?

Yes, before your first fill or refill, you will need to get a new prior authorization.

Is the EncircleRx program required for members globally?

Members living overseas who obtain their GLP-1s from retail pharmacies overseas are exempted from participating in the EncircleRx program. However, if you return to the U.S. and want to get the drugs here, you will be subject to the program’s requirements.

What GLP-1s does FSBP cover?

Bydureon, Byetta, Mounjaro, Ozempic, Rybelsus, Trulicity, Wegovy, and Zepbound.
