Our direct billing arrangement with Cleveland Clinic Canada covers routine annual physical exam*, chiropractor**, alternative treatments (acupuncture and massages)**, physical therapy*** and all covered outpatient medical services****.
Cleveland Clinic Canada opened in 2006 and provides in-person care in downtown and midtown Toronto as well as virtual care across Canada. Their services help their patients age well, move better, and know healthcare options. Their outpatient clinics are located at Brookfield Place and at Yonge and Eglinton. Additionally, their patients benefit from a multi-disciplinary team of Canadian physicians, wellness experts, and over 5,000 physicians and specialists across the Cleveland Clinic network.

*Routine annual physical exam: Member pays no deductible. FSBP pays 100% of covered services.
**Chiropractor and alternative treatments (acupuncture and massages): Member pays no deductible and any amount over $60 per visit (up to 50 visits per year). FSBP pays a maximum of $60 per visit and then all charges up to the Plan allowance; and all charges above 50 visits per person, per calendar year.
*** Physical therapy: Member pays applicable calendar year deductible and 10% coinsurance. FSBP pays 90% of covered services for 125 total combined outpatient physical, occupational, and speech therapy visits per calendar year for all three listed therapies.
****Outpatient: Member pays applicable calendar year deductible and 10% coinsurance. FSBP pays 90% of covered services.
